5W2H action plan for nonconformities

Sticky notes showing the seven fields of a 5W2H action plan: what, why, where, when, who, how and how much.

A 5W2H action plan for nonconformities turns a vague decision into executable work. It states what will be delivered, why it addresses the cause, where and how it will happen, who owns it, when it is due and how much it will cost.

Seven completed fields do not guarantee that a problem will stop recurring. A strong plan begins with evidence-supported cause analysis, produces implementation records and ends with effectiveness verification.

What 5W2H means

Field Question What to record
What What will be done? an observable deliverable
Why Why do it? connection to the cause or risk
Where Where? process, site, system or application point
When When? deadline aligned with risk and dependencies
Who Who? one accountable owner
How How? method, resources and expected evidence
How much How much? estimated cost or required resources

ASQ describes 5W2H as a structure for considering the different aspects of a process or problem. Within a QMS, it moves corrective action from a meeting into a traceable workflow.

Correction is not corrective action

A correction removes the detected nonconformity. Containment limits the immediate effect. Corrective action removes the cause to prevent recurrence. ISO 9000 distinguishes these concepts.

5W2H does not discover root cause. It organizes the response after methods such as the Five Whys and fishbone diagram have produced a conclusion consistent with the evidence.

Write fields that another person can verify

  • What: use a concrete deliverable, not “improve” or “monitor.”
  • Why: explain the causal mechanism instead of repeating the problem.
  • Where: identify the affected process, site, document or system.
  • When: consider current exposure, dependencies and interim containment.
  • Who: assign one person to coordinate progress and evidence.
  • How: state the method and expected proof of implementation.
  • How much: record money, hours or capacity required and compare it with the cost of poor quality.

Example for a document nonconformity

Problem: shipping documents were issued using an outdated customer instruction.

Confirmed cause: employees used email attachments because no official source blocked obsolete versions.

Field Entry
What publish the instruction in the controlled library and remove local copies
Why eliminate uncontrolled email attachments
Where shipping-document process at two exposed sites
When by September 12
Who document-control owner
How publish, replace links, communicate and inspect 20 records
How much 12 internal hours; no additional purchase

Implementation evidence includes the approved document, replaced links, communication and sample results. The effectiveness criterion could require no obsolete-version event for 60 days and two conforming sample reviews.

Implementation does not prove effectiveness

Completing every task proves execution. Effectiveness asks whether the cause stopped producing the problem. Define the criterion in advance using recurrence, a sustained quality indicator, a sample, an audit or a control test.

If the action is ineffective, do not merely extend a completed deadline. Reassess the cause, assumptions and action design.

How Konvéz Quality applies 5W2H

Konvéz Quality connects identification, root cause, action planning, implementation and effectiveness. Every action records what, why, how, where, who, due date and cost.

The workflow requires at least one action before it advances. A validator reviews the plan; each owner updates assigned actions and attaches applicable evidence. After all actions are implemented, Quality records the effectiveness decision and its rationale. An ineffective result returns the case to root-cause analysis for another cycle.

5W2H improves clarity, but the complete cycle produces the result: investigate, plan, execute, preserve evidence and verify that recurrence was prevented.

Frequently asked questions

What is a 5W2H action plan?

It details what will be done, why, where, when, by whom, how and at what cost, turning a decision into traceable execution.

Does 5W2H replace root-cause analysis?

No. Five Whys and fishbone diagrams investigate causes. 5W2H organizes actions selected after evidence supports the cause.

How do you verify corrective-action effectiveness?

Define an observable criterion, data source and verification period before execution. Completing tasks proves implementation, not necessarily effectiveness.
