Why record every deviation
A deviation that stays in a WhatsApp chat happens again, because nobody looks at the pattern. Recording the incident with evidence, an owner and a deadline turns the problem into information: you can see what happens most, what it costs and what has already been fixed.
ISO 9001, clause 10.2: react to the nonconformity, evaluate the cause and keep a record of the actions.
The path of an incident
Open
The person reporting describes the deviation, attaches evidence and sends it to the supervisor.
Awaiting supervisor
The supervisor approves or rejects it with a reason. If rejected, it goes back to open.
Awaiting Quality
Quality receives it and sends it for analysis.
Under analysis
Investigation and corrective action.
Closed
Closed with the corrective action taken, the person responsible and the date.
Day to day
Who operates
Records the incident on the spot, with a photo and a description, and sends it to the supervisor without having to put together a report.
Who leads
Approves or rejects it with a reason and tracks the amount involved and what can still be recovered.
Who audits
Sees the full path of each incident, from opening to closing, with the corrective action taken.
Everything that gets recorded
Suggested severity
Low, medium, serious or critical, suggested from the amount involved.
Financial impact
Currency, amount, exchange rate, recovered amount and whether it can still be recovered, with a deadline.
People and areas
People responsible for the deviation, units, departments and the supervisor for each case.
Secure evidence
Attachments scanned by antivirus, notes and a case file that can be sent by email.
Category and source
Configurable lists to classify incidents and spot patterns.

Several similar incidents can become a single nonconformity.

- Risks: 5×5 probability and impact matrix, inherent and residual risk, treatment with actions and deadlines, and alerts when a department piles up NCs.
- Suppliers: Weighted questionnaire, Gold, Silver, Bronze or Rejected rating and re-evaluation on time. Suppliers answer through a link, no login needed.
- Disputes: When an incident has a dispute, it gets its own handling, with technical justification, BL number and carrier fee.
- Recovered value: Financial control of incidents shows the amount involved, what was recovered and what is still open.
- TAPI: TAPI adds up each employee’s losses over 12 months and suggests an action and a deadline for each band.
- Management reviews: ISO 9001 9.3 input topics created automatically, participants, decisions that become an NC or OM, and minutes in PDF.
