Nonconformities
From root cause to effectiveness, without skipping a step.
Handle nonconformities with 5 whys or Ishikawa, a 5W2H action plan and an effectiveness check. If it didn’t work, the NC goes back to root cause.

Why treat the cause, not the symptom
Fixing the effect solves today’s problem; treating the cause keeps it from coming back tomorrow. A well-handled nonconformity goes through root cause analysis, actions with an owner and a deadline and, at the end, a check that the action worked.
ISO 9001, clause 10.2: determine the causes, implement actions and review their effectiveness.
The NC stages
Identification
What happened, where and with what evidence.
Root cause
5 whys, Ishikawa or both. There can be more than one cause.
Action plan
5W2H actions linked to each cause.
Implementation
Actions are carried out or canceled, and the effectiveness deadline is set.
Effectiveness
Effective, it closes. Not effective, it goes back to root cause.
Day to day
Who operates
Takes part in the analysis with 5 whys or Ishikawa and receives their own 5W2H actions, with deadlines.
Who leads
Tracks which stage each NC is in and which actions are overdue, without a side spreadsheet.
Who audits
Checks the cause, plan, implementation and effectiveness of each NC, with the evidence attached.
Built for real analysis
Ishikawa with 6M
Method, machine, material, environment, measurement and manpower.
5W2H per action
What, why, how, where, who, by when and how much.
Quality review
Quality can reject the root cause and the plan, and send the stage back.
Improvement opportunities
OMs follow the same stages and can come from audits and management reviews.
Linked to incidents
The NC shows the incidents that led to it.
Attached evidence
Each stage keeps its evidence for the audit.
- Which critical NCs are still open?
- Which actions are overdue?
- Notify the person responsible for this NC.
